The Incomplete Charts Report is a summary of chart documents that are still in progress or have been completed, but still need a client signature. This report also includes a summary of chart documents that are ready for approval when utilizing the Student Draft feature in Instinct.
When generated, this report will list all charts that are currently in progress, and/or unsigned as of the reporting period end date, regardless of the start date chosen. Any chart documents that have been Voided will not appear in this report.
Users need to have the analytics_incomplete_charts_report permission added to a role in order to access this report in Analytics.

The Incomplete Charts Report consists of the following:
Report Info Tab
The Report Info tab is the cover page for the report and consists of the Hospital Name and Hospital Location. It will also show what date/time range the report is for, when the report was created, and the time zone for the report.
Glossary Tab
The Glossary tab contains terms and definitions for column headings located throughout the report for clarity.

The Note at the top explains that the information in the report reflects the most recent information as of the end date of the reporting period, not when the report was created (ie. the data run date). The report includes all charts that were incomplete as of the end date of the report, therefore the start date does not affect what is included in the report. If a patient has checked out and there have been no updates to an incomplete chart since prior to the start date of the reporting period, it will still be included in the report since it is still considered incomplete as of the end date of the reporting period.
Terms listed:
- Patient ID – Patient ID associated with the in progress or unsigned chart
- Patient Name – Name of the patient associated with the in progress or unsigned chart
- Check In Time – Time at which the patient checked in
- Check Out Time – Time at which the patient checked out
- Reason For Visit – Reason for patient visit
- In Progress Chart Documents – Name of the chart that is in progress
- Unsigned Chart Documents – Name of the chart that is unsigned. Unsigned charts are those that are completed but have signatures required
- Ready for Approval Chart Documents – Name of the chart that is ready for approval
- Assigned Provider – Provider assigned to the patient
- Updated By – Staff member who last updated the chart
- Deceased Date – Date on which the patient was deceased, and blank for patients who are not deceased
In Progress Chart Tab
The In Progress Chart Tab displays a list of all chart documents that are currently in progress up to the end date of the report.

This tab consists of the following information:
- Patient ID
- Patient Name
- Check In Time of visit
- Check Out Time of visit
- Reason For Visit
- In Progress Chart Documents – the name of the chart document that is in progress
- Provider
- Updated By – the user that last updated the chart document
- Deceased Date – This will be blank if the patient is not deceased
Unsigned Chart Tab
The Unsigned Chart Tab displays a list of all chart documents that are currently completed, but still need to be signed, up to the end date of the report.

This tab consists of the following information:
- Patient ID
- Patient Name
- Check In Time of visit
- Check Out Time of visit
- Reason For Visit
- Unsigned Chart Documents – the name of all chart documents that are completed, but still need a signature
- Provider
- Updated By – the user that last updated the chart document
- Deceased Date – This will be blank if the patient is not deceased
Ready for Approval Charts Tab
The Ready for Approval Charts Tab displays a list of all student draft chart documents that are ready to be approved by the attending provider. Once approved, if the chart document requires a client signature, the document will appear on the Unsigned Charts Tab in future reports until a signature is obtained.

This tab consists of the following information:
- Patient ID
- Patient Name
- Check In Time of visit
- Check Out Time of visit
- Reason For Visit
- In Progress Chart Documents – the name of all chart documents that are ready to be approved by the attending provider
- Provider
- Updated By – the user that last updated the chart document
- Deceased Date – This will be blank if the patient is not deceased
